The Amba Wellness Coach website and mobile application are provided by Pfizer Inc. ("Pfizer"). This Privacy Policy ("Policy") describes how Pfizer ("Company" or "we") collects, uses, and discloses information obtained about visitors to the website ambawellnesscoach.com (the "Site"), users of the mobile Application (the "App"), and the services available through the Site (collectively, the "Services").

We respect your privacy and realize that the success of our service depends on the trust that users have in the way we handle their personal information. This policy sets out our commitments and explains the rights that you have with respect to your personal information. By visiting the Site, using or downloading the App, or using any of our Services, you agree that your personal information will be handled as described in this Policy. Your use of our Site or Services, and any dispute over privacy, is subject to this Policy and our End User License Agreement, including its applicable limitations on damages and the resolution of disputes. The End User License Agreement is incorporated by reference into this Policy.

If you do not agree to the terms of this Privacy Policy, including the collection, processing, and limited sharing of your health information, please do not set up or use the Amba digital wellness coach.

Your use of the Services is in connection with a program offered by Pfizer in conjunction with medication you have been prescribed prior to enrolling in this Program and receive from your Specialty Pharmacy Provider. Your Specialty Pharmacy Provider's handling of your personal information, including any health information, is governed by the Health Insurance Portability and Accountability Act of 1996 ("HIPAA") and your Specialty Pharmacy Provider's Notice of Privacy Practices, and not this Policy.

The Information We Collect About You

We collect information about you directly from you and from third parties, and automatically through your use of our Site or Services. Use of the Services requires registration. If you choose to register with us, you will be asked to provide certain information, including health information, and to direct your personal and health information to be used and shared as outlined in this Policy. It is your voluntary decision whether to provide us with any such information, or to direct the use and sharing of your information, but if you refuse to provide such information or direction we may not be able to register you to use the Services or your use of the Services may be limited.

The App will utilize your personal and health information from Specialty Pharmacy Provider that you direct Specialty Pharmacy Provider to share in order for Pfizer, and its service providers, to provide you the Services. Pfizer will not receive identifiable information from Specialty Pharmacy Provider except as described below in How We Share Your Information as it relates to adverse event information.

Information We Collect Directly From You. The information we collect from you depends on how you use our Services. To create an account, you must provide your name, date of birth, telephone number and email address. We also may collect additional information from you, including:

During registration or subsequently, you may choose to use certain features of the Services that will allow you to input personal information with respect to your health, such as the medications you take, the date of your prescription, the date you received your diagnosis, the number of refills you have made, how often you take your medication, the frequency you would like us to send you reminders to take your medication, whether you take your medication, and other information related to your treatment or your illness (“Health Information”). We will tailor our Services using this information.

Information We Collect Automatically.

We automatically collect information about your use of our Site and Apps through cookies, web beacons, and other technologies, including technologies designed for mobile apps. To the extent permitted by applicable law, we combine this information with other information we collect about you, including your personal information. Please see the section “Cookies and Other Tracking Mechanisms” below for more information.

How We Use Your Information

We use your information, including your personal information, for the following purposes:

To provide our Services to you, including to communicate with you about your use of our Services, to respond to your inquiries, and for other customer service purposes and to tailor the content and information that we may send or display to you, to offer location customization, and personalized help and instructions, and to otherwise personalize your experiences while using the Services.
To communicate with you about the Services and other related information, including email marketing.
To better understand how users access and use our Services, both on an aggregated and individualized basis. For example, we will evaluate which features of our Site and App are more (or least) used by users, and we will use this information.

Where we believe necessary to investigate, prevent or take action regarding illegal activities, suspected fraud, situations involving potential threats to the safety of any person or violations of our End User License Agreement or this Privacy Policy.
The App supplements pharmacy care, treatment and management services offered by the Specialty Pharmacy Provider. When you register for the Services, we will ask you to direct and authorize Specialty Pharmacy Provider to provide personal information about you, such as your medication list, prescription information, address, phone number, and other information related to your prescription to Pfizer’s service providers, so that the App can personalize services to you. The App will also provide Specialty Pharmacy Provider with your personal information in order to help coordinate your pharmacy care treatment and management and help manage your health. Pfizer will receive adverse event information from Specialty Pharmacy Provider to fulfill FDA reporting obligations. Pfizer will also receive de-identified and aggregated information.

How We Share Your Information

The App may share your personal information with Specialty Pharmacy Provider and Pfizer’s service providers for purposes of the Services. The App may share your personal information with Pfizer strictly for FDA-required adverse event reporting purposes. Information shared with Pfizer for adverse event purposes will be shared by the application to your Specialty Pharmacy Provider for follow up and then to Pfizer through the agreed-upon adverse event reporting, which is collected per government regulations.

Sometimes we may provide limited access to your personal information to the extent necessary to our service providers who are bound by an obligation of confidentiality, including vendors and suppliers that provide us with development services, technology, services, or content for the operation, development and maintenance of our Services or data and analysis on Services use. Such access is limited to the information reasonably necessary for the service provider to perform its services to us. We also require that such service providers agree to protect the privacy of your personal information and agree not to use or disclose personal information for any purpose other than providing us with products and services and as required by law.

Caregiver Access

You are able to designate a Caregiver to be the primary user of the App (a “Caregiver”) and to access the App on your behalf. By providing your Caregiver’s personal information, you affirm that you have the authority to share that data with us.

Our Use of Cookies and Other Tracking Mechanisms

We and our third party service providers use cookies and other tracking mechanisms to track information about your use of our Site or Services. We may combine this information with other personal information we collect from you (and our third party service providers may do so on our behalf).

Cookies. Cookies are alphanumeric identifiers that we transfer to your device’s hard drive through your web browser for record-keeping purposes. Some cookies allow us to make it easier for you to navigate our Site and Services, while others are used to enable a faster log-in process or to allow us to track your activities at our Site and Service. There are two types of cookies: session and persistent cookies.

Session Cookies. Session cookies exist only during an online session. They disappear from your device when you close your browser or turn off your device. We use session cookies to allow our systems to uniquely identify you during a session or while you are logged into the Site and Services. This allows us to process your online transactions and requests and verify your identity, after you have logged in, as you move through our Site and Services.

We and our third party service providers also use clear GIFs in HTML e-mails to our customers, to help us track e-mail response rates, identify when our e-mails are viewed, and track whether our e-mails are forwarded.

Third Party Analytics. We use Adobe Analytics, which uses cookies and similar technologies, to collect and evaluate information about the usage of our Site, and to the extent permitted, our Apps. We also may use other analytic means to evaluate our Site and Apps. Any personal information you provide on the linked pages or external mobile applications is provided directly to this third party and is subject to this third party’s provider’s privacy policy. This Privacy Policy does not apply to such other websites or applications, and we are not responsible for the privacy practices or content of any website or application not controlled by us. Security of My Personal Information

We have implemented reasonable precautions to protect the information we collect from loss, misuse, and unauthorized access, disclosure, alteration, and destruction. Please be aware that despite our best efforts, no data security measures can guarantee security.

You should take steps to protect against unauthorized access to your password, phone, and computer by, among other things, signing off after using a shared computer, choosing a robust password that nobody else knows or can easily guess, and keeping your log-in and password private. We are not responsible for any lost, stolen, or compromised passwords or for any activity on your account via unauthorized password activity.

Access To My Personal Information

You may modify personal information that you have submitted by logging into your account and updating your profile information.

What Choices Do I Have Regarding Use of My Personal Information?

We may send periodic promotional emails to you. You may opt-out of promotional emails by following the opt-out instructions contained in the email. Please note that it may take up to 10 business days for us to process opt-out requests. If you opt-out of receiving promotional emails, we may still send you emails about your account or any services you have requested or received from us.

Children Under 13

Our Services are intended for use by persons 18 years of age and older. Under no circumstances should the Services be used by children under 14 years of age, and we will not knowingly collect personal information from any person we know to be in this age group. If you are using the Services for the benefit of a child, please do not provide information relating to such child unless you have obtained the child’s parents’ or guardians’ consent, including their consent to this Privacy Policy. If you discover that your child has been using the Services without your consent, or that someone has been using the Services for or on behalf of your child without your consent, please contact us using the information below under “How to Contact Us,” and we will take reasonable steps to delete the child’s information from our active databases.

You may have certain rights regarding our processing of your personal information under applicable local state law, including for example, if you are a consumer of California or Virginia. If our processing and your personal information is governed by such laws, this section applies to the personal information and supplements the other sections of this Privacy Policy. This section does not address or apply to our handling of publicly available information lawfully made available by state or federal government records or other personal information that is exempt under applicable local state law, such as Protected Health Information held by Covered Entities, including Specialty Pharmacy Provider, or Business Associates under HIPAA.

We do not sell or share for cross-context behavioral advertising (as those terms are defined under applicable local law) the categories of personal information described below. We do not use or disclose sensitive personal information for purposes other than permitted under applicable local law.

We may retain your personal information for as long as needed or permitted in light of the light of the purpose(s) for which it was obtained and as outlined in this Privacy Policy, depending on (i) the length of time we have an ongoing relationship with you and provide the Site or App to you; (ii) whether there is a legal obligation to which we are subject; or (iii) whether retention is advisable in light of our legal position (such as in regard to the enforcement of the End User License Agreement, applicable statute of limitations, litigation or regulatory investigations).

As part of our verification process, we may request that you provide us with proof that you have been authorized by the individual on whose behalf you are making the request, which may include signed permission provided by such individual.
